Visitation of the Sick Request Form
Our parish has a Visitation of the Sick Ministry whose goal is to bring the Sacrament of the Holy Eucharist once a week to all parishioners who are unable to attend Sunday Mass because of illness or being home-bound. One day a month a priest and lay minister will visit and offer Sacraments of Reconciliation, Holy Eucharist, and Anointing of the Sick. If parishioners are going to the hospital for surgery, the parish office can arrange for anointing of the sick.
Due to recent legislation regarding patients' rights and healthcare privacy, hospitals are no longer permitted to give out the names and religious preferences of their patients without the explicit request of the patient or their family. Because of this, a priest or Minister of Holy Communion who has come to pray, visit or give communion may be unaware of other parishioners who have been admitted and would like a visit but have not notified the parish. We ask that parishioners help us remain informed about fellow parishioners who are hospitalized, or who are in a nursing home or homebound.
If you would like for a Extraordinary Minister of Holy Communion (EMHC) to visit a member of your family please complete the card below and return to the parish office. Once you fill in the pertinent information, you can fax the card back to St. Gertrude's at 513-561-7184 or return it to the Parish office (6543 Miami Ave, Cincinnati, Ohio 45243)
