This permission form applies to the following 2025-2026 Children and Youth offerings on the St. Gertrude Parish campus:
Faith Formation (Age 3-Grade 8)
- Catechesis of the Good Shepherd (Age 3–Grade 3)
- Sacramental Formation (Typically Grade 2)
- Family Faith (Grades 4–6)
- Confirmation (Grades 7–8)
Children & Youth Ministry Groups (Age 3–Grade 12)
- Little Flower Girls’ Club (Grades K–4)
- Junior High Youth Ministry (Grade 7–8)
- High School Youth Ministry (Grades 9–12)
